Job Description
About the role
As the Senior Clinical Care Partner, you will provide leadership and oversight across BallyCara’s Support at Home Program, ensuring clients receive high-quality care that is aligned to their goals, assessed needs and funding.
This is a full-time role, working Monday to Friday located at our Kippa Ring office, reporting to the Integrated Community Manager who is based in our Melbourne office.
You will be well supported as part of a connected Support at Home team, while also having the autonomy to manage your day-to-day work locally and independently as required.
This is a senior clinical role that combines care oversight, team leadership, service coordination and clinical governance.
You will work closely with Care Partners and the broader multidisciplinary team to support integrated service delivery across nursing, allied health and care supports, while ensuring services are delivered in line with aged care legislation, quality standards and BallyCara’s values.
While this role is not intended to hold a full caseload, there may be times where you provide direct client support or manage a small caseload to support leave coverage or service continuity.
What you will be responsible for:
Leading clinical oversight across BallyCara’s Support at Home services, ensuring safe, person-centred and goal-focused care
Supporting and mentoring Care Partners and the wider team to deliver high-quality outcomes for clients
Overseeing complex referrals, care planning, reviews and service coordination across nursing, allied health and care supports
Monitoring client funding, care management activity and service delivery to ensure compliance with program requirements and client goals
Driving clinical governance, documentation quality, continuous improvement and alignment with aged care legislation and quality standards
Building strong relationships with clients, families, internal teams and external stakeholders to support coordinated care and service growth
Desired Skills and Experience
About you
We’re looking for an experienced senior clinical professional with Registered Nurse qualifications to lead high-quality care and support older people to live well at home.
You bring a calm, practical leadership style, sound clinical judgement, and the ability to balance person-centred care with quality, compliance and operational oversight.
You will also bring:
Current AHPRA registration as a Registered Nurse
Strong knowledge of the Support at Home Program, aged care funding and reporting requirements, and the Strengthened Aged Care Quality Standards
Experience in clinical assessment, care planning, referrals, case management and supporting complex client needs
Demonstrated leadership experience, including coaching, mentoring and supporting team capability
Excellent communication and relationship-building skills, with the ability to work effectively with clients, families, staff and external stakeholders
Strong organisational skills, including the ability to coordinate services, manage competing priorities and maintain high-quality documentation
Sound judgement, resilience and the ability to navigate change, identify risk and solve problems in a practical and client-focused way
