What you’ll be doing
You’ll be at the heart of each client’s journey—supporting them from day one and beyond.
Lead onboarding and clinical assessments for new clients entering Support at Home
Develop personalised, goal-based care plans that promote independence
Manage a caseload / funding with a focus on wellness, reablement and restorative care
Coordinate services across nursing, allied health and support teams
Provide clinical oversight to ensure safe, high-quality care at home
Build trusted relationships with clients, families, GPs and providers
Desired Skills and Experience
About you
You’re a confident, compassionate RN who thrives outside the traditional clinical setting.
You enjoy autonomy, love building relationships, and take pride in delivering care that truly centres the individual.
You’ll bring:
Current AHPRA registration as a Registered Nurse
Experience in aged care, community care, or case management
Strong clinical assessment and care coordination skills
A passion for wellness, reablement and independence
Excellent communication and relationship-building ability
What you’ll need
Current Driver’s License
First Aid & CPR
Police Check (or willingness to obtain)
Right to work in Australia
