Job Description
About the role
As a Community Workforce Team Leader, you’ll lead and support a team of Support Workers delivering high-quality home care services across the community. You’ll balance people leadership, client relationships, rostering, problem-solving, and quality outcomes—no two days are the same.
This is a hands-on, role suited to someone who enjoys multitasking, resolving issues calmly, and keeping services running smoothly.
What you’ll be doing
Oversee the accurate and compliant management of client files, ensuring all documentation meets CHSP quality and audit requirements.
Coordinate and complete annual client reviews, ensuring individual needs, goals, and care plans are up to date and aligned with program guidelines.
Lead the onboarding process for new CHSP clients, including initial assessments, service setup, and communication with clients, families, and stakeholders.
Lead, support, and guide Support Workers to deliver exceptional care
Manage day-to-day workforce issues, including conflict resolution and service troubleshooting
Work closely with Care Partners and internal teams to meet client needs
Contribute to recruitment, onboarding, performance, and quality outcomes
Use systems confidently to manage records and service delivery
Desired Skills and Experience
What you’ll bring
Experience in a client-facing aged care or community services role
Strong conflict resolution and communication skills
Confidence working in a fast-paced, high-volume, multitasking environment
High level of computer literacy and confidence using systems
A proactive, calm, and people-focused leadership style
Role Requirements
Current driver’s license
Satisfactory police check (or willingness to obtain through BallyCara)
Right to work in Australia
Cert IV or higher in Aged Care or a related field (desirable)
